Freestanding mausoleum, dated 1838, on a rectangular plan. Limestone flagged "slated" roof. Repointed broken coursed rough cut limestone walls with clasping buttresses to corners. Four centre-headed door opening. Set in grounds of Saint John’s Church (Knockainy).

This mausoleum houses the remains of the O'Grady family and is an important element of the funerary architecture of County Limerick. The mausoleum is similar to the mausoleum housing the remains of the Westropps and the Stacpooles at Saint Beacon's Church (Kilpeacon) (see 21902233). A panel is inscribed: ""THIS TOMB WAS ERECTED BY MARY/WIDOW OF EDWARD O GRADY ESQr./OF STEPHENS GREEN DUBLIN/FOR HIM AND THEIR/IMMEDIATE DESCENDANTS/1838"".
